PROSTATE CANCER:
We offer the latest in diagnostic and therapeutic regimens for prostate cancer. We use the latest in ultrasound technology, including color flow and power Doppler. Dr. Chinn personally reviews all pathology slides with his pathologist, Dr. Ken Seager, who excels in prostate cancer, and never hesitates to order special stains, or second opinions if required.

For prostate cancer, we believe in procedures that are minimally invasive, successful, with low complications, and best of all, can be repeated.

HIGH INTENSITY FOCUSED ULTRASOUND (HIFU):
Dr. Doug has begun to treat patients with a newer technology, HIFU. Some patients say that it should not even be called surgery, and the NIH calls the procedure non-invasive.

Prostate:
Dr. Doug feels that HIFU should be the first choice therapy for failed radiation cancer patients, and that HIFU may offer the best chance for potency. The major drawback to HIFU is that it currently is not FDA approved in the USA, and therefore must be performed outside of the USA and the costs must be covered by the patient.

HIFU can also be used for BPH, and Dr. Doug has already treated one patient who is doing well. Although cryosurgery works very well for BPH, HIFU may be even better since it really is non-invasive.

For a primer on HIFU, see PCRI.org, Feb 2005 newsletter

Breast:
Dr. Doug believes that HIFU potentially offers great promise in the treatment of benign and malignant breast lesions. Dr. Doug hopes to begin or participate in the development of HIFU applications for breast and other organ systems.

CRYOSURGERY:
Dr. Douglas and Mahlon Chinn have been pioneers in the role of cryosurgery for treating prostate and renal cancer. We have been performing prostate cryosurgery since 1993, and have presented data worldwide and have been co-authors in peer reviewed journals and in a textbook. Dr. Doug developed and patented the use of temperature monitoring for prostate cancer. Dr. Doug has trained physicians world-wide in prostate and renal cryosurgery. Dr. Doug has also pioneered the use of experimental cryosurgery in breast, uterus, and benign prostatic hypertrophy.

Prostate:
In order to preserve potency, Dr. Chinn has performed focal therapy since 1993, These patients will require saturation biopsies prior to focal therapy. Dr. Doug has a new device in development that may spare the nerves, yet totally ablate the entire prostate gland.

Rectal injury is very rare in cryosurgery, and although we were the first to inject saline between the prostate and rectal wall in 1993, we seldom have the need to perform this maneuver during cryosurgery.

As the primary treatment for prostate cancer, cryosurgery has an extremely low incidence of any type of urinary incontinence, and total incontinence is very rare. However, for radiation failure, the risk of incontinence does increase.

For very large benign glands without cancer (BPH) cryosurgery works very well also. BPH cryosurgery is FDA approved, but not covered by insurance.

Renal:
We also have experience in treating kidney tumors, since 1999. We initially treated renal tumors with an open surgical incision. We now do almost all kidney cryosurgery percutaneously (through the skin) using CT guidance.

Other:
We have also done clinical experimental work on breast lesions and sarcomas. We believe that cryosurgery has the potential to treat many other benign or malignant conditions, including bone lesions.
